In the box above please include the following information :
1. The child's name (typically first and middle, but you can do first and last), time of birth, weight (in pounds and ounces), inches, and birth date.
2. Select a picture from our clipart selection which you can find in the product photos, or if you have a HQ scan of the baby's footprint you can send it to us and we can include it in that space (the sample photo is pictured with a scan of the foot print).
